When liposuction is performed underneath the chin, it is known as submental contouring. In this procedure, one or two small incisions are made on the underside of the chin and/ or in the crease around the earlobes. Thereafter, a cannula is inserted beneath the skin and into the fatty tissue to break up and suction out the excess fat. The skill and judgment of cosmetic surgeon while performing this procedure is critical as taking out too much of the neck fat can result in unwanted results.
When a layer of muscle situated on the each side of the neck known as the Platysma becomes loose and there is an occurrence of "banding" or vertical cords, the procedure that is carried out to tighten the superficial neck muscles is known as Platysmaplasty. This procedure can be carried out in insolation or performed in conjunction with liposuction to reduce excess fatty tissue. This procedure, when performed in isolation, can easily be accomplished through a small incision on the underside of the chin. The tightening or repositioning of the underlying tissues makes it possible to achieve dramatic results without putting any tension on the skin or creating a pulled look.
Your plastic surgeon may want to further support your neck with sutures, mesh or AlloDerm, to achieve contouring or desired results in the neck.

Changes of the Neck with Ageing:
Different areas of the face and neck are affected differently by the ageing process. We usually see the following changes with ageing of the neck:
Loose, flaccid skin under the chin requires hardening with a laser. If the skin is too excessive, it may have to be removed with excision surgery. If you have visible vertical stripes on the neck, they will have to be tensed in the midline of the neck muscles. Deposits of fat in the neck can be removed with liposuction or direct excision.
